On Wednesday, May 8, we were joined by guest speaker Dr. Jamie Chang, Chief Clinical Operations Officer at CHA Hollywood Presbyterian Medical Center, to discuss CMS' Bundled Payments for Care Improvement Advanced (BPCI Advanced) program.

Jamie Chang, MD, MBA, FACEP, is an emergency medicine physician and clinical operations consultant. He supervised the design and implementation of HPMC’s Continuing Care Program, which aligns incentives to manage patients post-discharge as part of the hospital’s BPCI Advanced initiative.

HPMC worked with Avant-garde Health to evaluate and maximize their performance in the program. Initial participation results include substantial improvements in both readmission rates and post-acute care utilization and costs, resulting in projected $3M reconciliation payments.
